Transster.com is a website that archives photos of all types of surgeries for FTMs, The surgeon I'm probably going to choose is Dr. Gary Alter (you might recognize him from the FX show "Dr. 90210). One of the most famous FTM chest surgeons is Dr. Michael Brownstein in San Francisco, California. I haven't had surgery yet so I can't really speak from experience. Depending on chest size, there are different procedures that can be done. Michael's site outlines it very well : here are the different types of chest surgeries.
 

As for funding, some insurances cover it, many don't. I've heard of FTMs taking out student loans to finance their surgery ... I don't think it's legal and wouldn't recommend it. I have money in the bank account from family members, I just need to save up a little bit more before I reach my goal. I've tried going the parental route because my parents are loaded and could pay for the procedure instantly, but they completely disagree with my choices and wouldn't ever consider it. So, it's the work  and save money route for me.
